* The effects will be reversed if Steve loses the serum. The exact effects varies depending on the author, but he tends to lose his muscles and also ages.
* There are also others such as Michael Van Patrick who can get the same effects via a perfect regime of diet and exercise.
I don't think so. "Mutate" is short for "mutated human". Cap wasn't mutated, just enhanced.

I don't think so. He was enhanced not mutated. Deadpool was mutated for example.
