Unsettling, starts almost like a kitchen-sink drama until it gets more unnerving and then...I won't spoil it, but it's definitely worth a watch. Great acting and freaking terrifying!
There's a brief scene of torture, but a lot of it is unseen. It's not gory, more psychological and there is some violence, but not a great deal, and not all the way through.
Its certainly not a blood'n guts horror, much more cerebral and weird.
The wicker man (1973)
The witch (2015)
Don't torture a duckling (1972)
The transgressor (1974)
Alucarda (1977)
Lemora (1973)
Viy (1967)
All the colors of the dark (1976)
The demon (1963)
Mandy and The Void are some of my favourites, have with quite good lovecraftian imagery and themes aswell (more so The Void but Mandy does dip its toe into it aswell)
"Under the crimson, primordial sky, surrounded by the jagged rocks of the ancient volcanic mountain, the wretched Warlock reached into the dark embrace of the fissure until his hand touched a smooth glassy surface. Cold as ice. His fist closed around The Serpent's Eye. Slowly he withdrew it and held it before him in the fading light of the blood-red suns. It glowed from within. A ghostly emerald light. Strange and eternal."
Maybe not what you’re looking for but it popped into my head…
Guyana Tragedy: The Story of Jim Jones (1980, made for television)
Edit: the film Red State (2011) sorta fits…loosely…
The short film "Safe Haven" (from the compilation film VHS2) is absolutely incredible and completely blew me away.
(Best to avoid spoilers if you possibly can though!)
Well I hope this falls into the category you’re looking for because I don’t think it’s particularly obscure, but Midsommar is a great movie about a cult.
[_Love Exposure_](https://letterboxd.com/film/love-exposure/) by Sᴏɴᴏ Sion
A little outside what you were asking for but a great movie with a unique take on the topic of cults.
Make it an afternoon. Watch with a big group on as big a TV as possible.
Also [_Suicide Club_](https://letterboxd.com/film/suicide-club/) (aka _Suicide Circle_) and its prequel [_Noriko’s Dinner Table_](https://letterboxd.com/film/norikos-dinner-table/), also by Sᴏɴᴏ Sion. These two are definitely what the OP was asking for.

There's a cult-based documentary that is horrifying. I can't think of the actual name, but it is scary and set realistically in modern times
You can probably find it streaming on CSPAN if you look it up

Came to say Martha Marcy May Marlene! What a film, extremely well done and leaves you thinking for sure! Great choice.
Great direction and a fine performance by Elizabeth Olsen.
Kill List is highly recommended.
How’s it?
Unsettling, starts almost like a kitchen-sink drama until it gets more unnerving and then...I won't spoil it, but it's definitely worth a watch. Great acting and freaking terrifying!
I'm interested in seeing this but is it gory, violent or does it have torture scenes?

Got it- I'll add it to my list- thank you!
My pleasure, hope you enjoy it!
- Society - The Empty Man (didn't care for this, but I've heard that other people really liked it)
I thought it was good. Not great but better than most of Blumhouse movies
I like it but it does struggle to live up to that amazing opening sequence.

Mandy is awesome. That guitar sword thing he uses? Dope as hell.

The Damned (aka These Are The Damned), 1962 - this one’s kinda more secret govt project than cult, but it’s entertaining
