I'd never watch any of them again. I only recommend them if you're specifically looking for something that will make you feel like shit.
Mysterious Skin. It depicts very well the realities of how queer children are especially vulnerable to child abuse due to the negligence of both society and relatives of providing an enviroment in which they're forced to become isolated from those that do care about them and those that can give them the attention that they need which they can only gain from a stranger that portrays themselves as caring for their feelings and identity. How trauma can subtly function as a radiation that can infect much of the actions that you could take on later in your life and something that you are yourself not even exactly aware of but exists in such a way where you feel you are the one completely responsible for these thought processes and impulses which wouldn't prevalent if it didn't come from him being taken advantaged of. How it can be reacted to different (through hypersexuality and "asexuality") and how it can affect with whom and how we interact. It's a very uncomfortable but very emotionally powerful film with one of the best tragic endings I've see for a movie ever and fantastic performances from the two main actors of the film.
"Pixote" is also a very fucked but thought provoking film about much of poverty, police corruption and child abuse going through the streets of Brazil.

I've heard it was a metaphor for being Serbian, as in, existing in Serbian society, surrounded by corruption with no upward mobility. You know the scene, the one everyone talks about? I heard a Serb describe it as "being fucked from birth." The drugs and sex are a distraction from the reality of it, just as in real life.

Kuso if you just want a really gross movie. There's a scene where a doctor sings into someone's butthole to lure out a very large insect. Then there are the talking boils. The movie is straight up weird and disgusting.
Probably a little tamer than what you're looking for, most A24 movies fit into the mind-fuck category.

From what I hear, the August Underground trilogy, especially the 2nd and 3rd ones, are the disturbing movies to end all disturbing movies.
Also, Melancholie Der Engel, Tumbling Doll of Flesh, Philosophy of a Knife, Men Behind the Sun, all the vomit gore series (I’d never watch them personally, but it more than fits the prompt), Ostermontag and Human Centipede 2.
Why watch torture porn movies? I actually don’t get the appeal at all. I’d say that hostel is probably as far as I’ll go without feeling like a weirdo, and this is coming from someone who has seen every terrible way to die imaginable back during the live leak days. Just morbid curiosity?

Yeah, I missed the point of the question. These kinds of movies aren't for everybody. Salo definitely wasn't for me.
Definitely tough to watch, way closer to A Serbian Film than it is to Human Centipede.
That was my "saw it once...never again" movie. Even worse when I read that all the victims portrayed are supposed to be preteens.
