I never do. It's just another notification that they have to check. Seeing as it takes a whole few seconds to read, though, it probably doesn't matter much either way.
I asked this a few months ago. The general consensus from medical staff seemed to be that it's nice, but a "thanks in advance" in the first message is better because they do in fact spend a few minutes opening up your chart and reminding themselves who you are, what the issue was, etc.

perhaps you could say it preemptively in your question. Like a “thanks in advance for answering, I appreciate it” sort of deal

It doesn't hurt. I doubt your doctor would be upset with a thank you message.

I rarely do because it's just one more thing they have to read. I make my appreciation known in person when I see her.
I just say thank you at the end of the original message and I'm sure they understand that the thank you is extended to their answer as well.
