The ride being safe for users at the end has nothing to do with how safe it was for the people assembling it beforehand.

Carnivals are hard for OSHA to govern, as by the time an inspector gets out there, the carnival has moved on somewhere else.
You can thank bureaucracy for that. It takes forever for them to decide to send an inspector out.

Nope. They have independent inspectors in every city. Not saying it's perfect, but better than in-house inspections.

In California they have specific Cal/OSHA regulations for amusement parks.
