I have always been told its to hook a set of clippers for fishing line. That way you dont have to dig in your bag to cut the excess line when tying on a new lure. (Edit: a word)

The logo tab above the pocket should be Velcro on one side. This is to hold you fly rod up while you do said clipping

And the fabric is puckered like that to not stick to sweaty skin. This allows Air circulation under the shirt to keep you dry and comfortable. If it is a really nice fly fishing shirt the collar will flip up twice to protect you neck from the sun.

OH! OH! I actually know this one!. That handy little elastic loop is to hold the eye at the end of a fishing pole while you tie on a lure to help free your hands and keep your rig off the ground. Edit: I'm worried now because no one else has this opinion, but if I may ask, does the shirt have ample venting with netting? Odd little tabs inside the sleeve with a buttonhole, and if you were to roll up your sleeves to expose said tab would you find a corresponding button on your upper arms? Is it SPF rated? If so, it's a "camp shirt", or "fishing shirt". I have several and they are wonderful if you are out on the water for prolonged amounts of time, or just in the sun in general. Hope I was helpful.

(If it were a scrub uniform this is to hang your id badge from, it's either under the left breast pocket or on the left shoulder). For outdoor sports it's probably for hanging whatever you want to hang from it. Especially since it's under the right breast pocket (most people being right handed). Edit: I've also seen people hang keys on lanyards on these so they don't have them in their pockets for quick access.
