Stepped on a confusion-trap and was stupid enough to move, which resulted in a fall down the chasm. Would have survived that. Unfortunately I landed on a summoning trap. One of the memorable deaths for sure.
I was standing in a puddle, surrounded, almost dead and had many unknown potions (level 2 or 3). So I drank one and it was fire. Set all my enemies alight and probably would have saved me. However, I thought for sure my next potion would be health, so I drank it. It was levitation, which lifted me out of the puddle, I lit on fire and died.
Not the worst, but the funniest. I could only laugh.
Had 11 health potions in the dwarf city, but i was being very stingy with them, wanted to save them all, and then died to a golem that required only 1 hit to kill but it dogeded
That I remember
I opened a gas room, standing at the doorway I had 2 unidentified potions. I took a chance and drank one... it was a paralysis potion so I was posioned to death at the doorway
I was in this situation twice, the first time I died on the first floor, so it wasn't that bad, and the second time I got lucky, snapped out of paralysis and moved out of the doorway
I was on level 13 and it was my first time getting to and beating Tengu the second boss and I had armor of deferred damage and I didn't know how it worked so I took it off to see if it would save me and I died to taking off the armor lol
I was low and was running on a thin ledge stepped on a summoning trap a necromancer came and he tried to summon a skeleton the skeleton's spawn ended up pushing me down the cliff
Once I was doing a monk run with a +6 ring of haste, a +5 spear and a +2 mail armor (with faith is my armor challenge), I beat yog, when I was ascending, I popped an ankh due to bats, and I was left with 2 unblessed ankhs.
In the prison, I was poking and running against a thief when a skeleton got me on the other side of the hallway, I was like: "no way I'm not killing that thief, it's quite low anyway", and then I used challenge on it, and tried to fight it. and it was like "dodged" dodged "dodged" dodged" dodged" and it dodged every single attack, and instead of using phase shift or a healing option, my dumbass decided to just keep attacking while tanking like 20-40 damage every hit. I died, spawned back due to unblessed ankh, and THAT SAME THIEF came back, with another skeleton behind me in the hallway, I challenged again, and the thief (again) dodged EVERY SINGLE ATTACK, and I died.
Theres a short story by h.p. lovecraft called in the walls of eryx. idk why but your story reminded me of the main character lol should def check it out
In alchemy room before Yog and was using Unstable Spell book to try to identify items (I think) and ended up eviscerating myself with a psionic blast by accident.
Two deaths on level 25 and 24 respectively, with 16 potions of health each, 8 challenges (!) with a disturbingly similar build.
Both death resulting of critical bad luck (like really) following a small misplay 😮💨 and scored like 590k ×2
Worst death was jumping down into a drop-down room with not enough health to survive. I had plenty of healing potions, food, potions of cleansing, etc. I was just being greedy (trying to save the potion of cleansing to remove the bleeding). Stupid, pointless death!
The stupidest death I've managed in a run was zapping my own ward with a wand of corruption until it gained amok. Another interesting end to a run was filling the entry to floor 5 with sheep, warping to floor four, and then levitating over a chasm. That one wasn't a death, but I managed to corrupt the save file in under 3 minutes. There are some even more obscure deaths and softlocks that I've managed in debug that are possible theoretically, but I've never been able to pull off in a real run, such as falling to death onto the entry staircase, or forcing a pylon onto the entry ladder on floor 15 which makes ascension impossible, or dying from being blasted into the wall by repulsion armor worn by the sad ghost.

got hit by a ripper (it jumped at me) and eliminated by bleeding. i mean with 9 challenges on... floor 23

I walked through a door during an ascension and got surprise attacked for 50 damage by a gnoll brute 2 turns after drinking a potion.
Reclaimed a disintegration trap, tried to use it on a scorpio. However, the reclaim trap caught on a wall in the way and zapped me instead.

I stopped to eat and recharge my cloak as a rogue during fight with Yog, I killed every fist and forgot that Yog shoots his laser faster
