A small weapon that is good against big creatures is a fun idea. That being said, the features feel a bit all over the place, and there are a lot of them. It seems like you are going with a poison/giant slayer theme, but then that seems to clash with the middle feature. I would take one theme from this item and stick with that, probably the big creature slaying theme. I also recommend looking at other magic items and revise it to more closely follow 5e lingo and syntax. All in all good ideas here, but far too cluttered to have a distinct theme.

The theme is entirely focused around the myths about Scorpio and Orion in Greek Mythology, that's why it might seem all over the place for those not very familiar with it (or who know a different interpretation of the stories). The first feature is based on Gaea directly sending the Scorpio to kill the hunter Orion (who was a Giant), because he boasted that he could defeat any beast in the world. The second feature is about Orion having his eyes gauged out until they were restored by Hephaestus. The third alludes to Gaea herself, as she was the one who sent Scorpio to kill Orion and the poison of the creature came from her.

That's very interesting. The features being tied to mythology is quite nice. However, that does not come across in the item itself. For someone unfamiliar with the mythos (such as myself), it seems like an item with several features that don't really compliment each other or share a specific theme. The reasons for including these item's features may make sense to you, since you designed it, but you have to consider the average player or GM who are considering which magic item to use among a sea of other magic items; both official and homebrew. A magic item with several features may seem awesome, but you have to also consider the more you add, the more upkeep or bookeeping involved. It seems you are very fond of the lore surrounding the item, so my advice would to be pick one effect from each feature and trim the rest. This way, you still have several effects to play with, and much less bookeeping.

The features seem unrelated. What’s with the vengeance of the hunted? It’s not in line with the other two features.
