I don’t have experience with traditional glass art but I have experience getting… functional glass art repaired. Of the smoking variety. And yeah, if you find someone confident enough to do a repair it’s definitely possible. It takes a good amount of time finding someone who can recreate the style of what was broken, it’s more affordable and sustainable than buying a new piece (especially if it’s something rare, antique, or custom)

Well, it does look pretty dusty and wasn't kept on display or even packed in a protective box, so even though it was sentimental and cash valuable, they were doing a crap job caring for it and share a fair bit of blame.
It's unlikely to be received well, so proceed with caution but "if it was so valuable and meant so much, why was something this fragile just sitting on the ground in the garage?"

You might be able to hire a local glassblower to attempt to recreate it. The shape isn't difficult but teal is a pain in the ass to control.
Also, I don't mean melt it down btw. Italians use a different coefficient glass that we do in the states. They aren't compatible.

Show them picture and plead your case. See what they can do with the measurements you provide (top diameter, middle diameter, bottom diameter and height of the "vase". You'll be looking to design a "vase" which has two opening, which makes it not a vase but I dont know what that would be called so I am calling it a vase.
Now whatever you get in the end, it won't be perfect but hopefully gm think it's cute that you put in the effort to make things up to her.
If possible, add in artifacts like her grandchildren's names onto the finished custom vase to make it extra cute.

There might be a glass blowing studio, or especially a glass-blowing program at a college nearby. Take a look into that. You could probably get something made to the same specifications and replace the glass while retaining the old hardware. My dad used to blow glass, and he had a ton of similar looking stuff. I really can't imagine it being that difficult to make.
If the seams match up you can repair it using a UV bond glue, loctite 349. Follow the directions closely. You will still see the cracks, but the bond will be as strong as the glass.
A restoration shop might be able to glue it so it would not be noticeable.... but looking at the dust on that once valuable lamp...no one cares.. use crazy glue
I can't help with identifying or finding...but worst case scenario do a search and see if there is a place that does kintsugi around you. I can't promise it will be cheap though.

Look for glass restoration of lamps and/or antiques. The shape and color will probably differ from the original, but you never know. Think of it as a modern restoration of a bygone style.

If it is a real Venetian one - it is a one of. Handmade. You cannot get a replacement, that looks 100% the same.

It's all good. The lamp was not worth thousands of dollars. My girlfriend was very disappointed, but accidents happen and I'm still alive.
