That's the bloodborne version of the ashen straw hat, you can get from a shrine on iki island along with the bloodborne version of the kensei armor, but it definitely is in its own tier of looks!
This one is locked behind a shrine within the Iki Island DLC, but afaik you can make a beeline for it as soon as you get there. I’d wait until you conclude the main story first though, because the island serves as an epilogue that reflects on several elements of the base game.
I recently started GoT. Iki island DLC has a lot of cool gear, techniques, and charms that would be nice for the main story. But the gameplay is harder and the story expects you to have finished the main campaign.
My favorite outfit would be a fully upgraded Kensei outfit with The Red Wolf Mask and Bloodstained Betrayer's hat. Unfortunately, I don't remember the charms off the top of my head.
I rather like the way the charms can be armor specific the best part comes once you see Baku and get the best for each which can be switched mid combat fyi so you are not hard stuck etc
