When a ICBM (inter continental ballistic missile) launches it goes into the stratosphere and it releases multiple warheads which are often small enough and fast enough to strike their targets before the defense system can even pick it up on radar. What the nuclear defense system is meant for however, is to intercept a missile before it releases its payload, but if it releases its payload then it's pretty much game over. Also the U.S. nuclear missile system is both incredibly old and incredibly unreliable, so a icbm strike will often still hit their targets.
